Action Plans
It is very important to fill out the following forms if you child has a medical condition. These forms allow us to communicate in a simple way to staff who may be taking care of your child so that we know what to do in an emergency.

Medication Authorization Form
If your child requires medication at school (over the counter or prescription) a form must be filled out and signed by the parent. Prescription medications must also have a physician signature. Forms may be faxed back to your child's school.

Immunization Schedule
Below you will find a copy of the State of Ohio's Immunization schedule. Students must show proof of up to date immunizations or exemption. 

Immunization Reminder for Parents of 7th and 12th Graders:
The Ohio Department of Health requires all 7th graders to have a Tdap vaccine and a Meningococcal vaccine prior to the start of school.  All 12th graders are required to have 2 doses of the Meningococcal vaccine (only one dose is required if it is received on or after the 16th birthday).  

It is advisable to make an appointment for your child to receive these vaccines to avoid exclusion. As soon as your child receives this vaccine, please provide proof of immunization from your physician to your school office so your child’s school health record can be updated.
