Dear Madison Local School District Families, We are writing to inform you that FinalForms, the platform we use for registration and compliance, recently identified and responded to an incident involving unauthorized access to a small number of user accounts in our district and many others. The issue was promptly investigated and contained. It’s important to emphasize that no student data remains at risk. The incident was carried out by a lone actor who gained access using legitimate login credentials previously compromised in unrelated, external breaches (such as PowerSchool, Chegg, Yahoo, etc.). These credentials were the same as those set by the account owner in FinalForms. The incident did not result from any vulnerability within the FinalForms platform. FinalForms identified the source of the unauthorized access, isolated the lone actor using a controlled environment, established direct contact, and obtained a full confession. During a live, recorded session, the individual permanently deleted the accessed data under FinalForms' supervision. FinalForms continues to actively monitor all systems to ensure the ongoing security of student information. No action is required on your part at this time. FinalForms and our district remain fully committed to protecting the privacy and security of student data in compliance with all federal and state requirements, including FERPA and the Student Data Privacy Agreement (SDPA). 100 Years of Madison Local Schools Carnival Are you ready to celebrate 100 years of RAM Pride? To cap off the 100th Year of Madison Schools the district will be hosting a 100 Years Carnival on Saturday, May 17th from 12pm-3pm behind the Madison Middle School! The carnival will include games, inflatables, multiple food trucks, and other fun activities. We encourage you to join us as we celebrate 100 Years of Madison Schools!
