Madison Local Schools will be accepting Open Enrollment Applications from
May 1, 2025, through May 31, 2025, for the 2025-2026 School Year.
Please complete the online Google form below or pick up a paper form at the Madison Board of Education.
DEADLINE FOR ALL APPLICATIONS IS MAY 31, 2025.
Intra-District –From within the School District (Madison South to Eastview/ Madison South to Mifflin/ Eastview to Madison South/ Eastview to Mifflin/ Mifflin to Madison South/ Mifflin to Eastview)
Inter-District - From Outside, ANY residency in Ohio School District (Mansfield, Lexington, Lucas, Galion, etc.)
Inter-District Renewal Open Enrollment Applications MUST be accompanied by an updated proof of residency. Proof of residency can be returned to your child’s school building, the Board of Education, or emailed to Allison Conn at aconn@madisonrams.net. If emailing, please include your student's name and grade, and ensure that documents are readable.
New Inter-District Open Enrollment applications MUST be accompanied by the following copies. Incomplete applications will not be accepted. Documents can be dropped off at the Board of Education or emailed to Allison Conn at aconn@madisonrams.net. If emailing, please include your student's name and grade, and ensure that documents are readable.
Student's up-to-date immunization record
Student's birth certificate
Proof of residency at current address (within the past 2 months) in the custodial parent’s name.
Acceptable documents are:
A service/utility bill (gas, electric, rent receipt, cable, trash, telephone, or water bill).
A voter registration confirmation card is also acceptable.
No other “proof” is acceptable to meet this requirement.
Custody papers, if applicable.
A Current IEP (Individual Education Plan) if the student is receiving special education services.
Current ETR (Evaluation Team Report) if the student is receiving special education services.
